About Emma

Emma Reeve is the Director of Serious Risk Prevention (SRP) and founder of EMLR Consultancy.

With over 20 years’ experience in adult social care, from frontline roles to senior leadership, Emma is an operator who works on site, tests practice, reads culture, and turns findings into actions that change outcomes.

Emma’s passion comes from seeing the human impact behind every incident: the families who deserve answers, the teams who want to do their best, and the leaders who need a real picture of what’s happening on the ground. This drives a commitment to honest insight, practical solutions, and creating safer services where people’s lives and wellbeing come first.

Why SRP Exists

The same high-harm risks keep causing preventable deaths

Policies look fine; practice falls apart in real moments

SRP exposes where risk control actually work and where it does not

Deaths and prosecutions in social care aren’t rare outliers. They’re predictable when controls slip, and when they land the impact is brutal: loss of life, public judgments, regulatory action, personal accountability, and fines that routinely reach high six figures and, in the most serious cases, seven.

Insurance costs rise. Ratings fall. Commissioners hesitate. Value erodes. SRP exists to make your controls work in real life, not just on paper.

What We Do

Incident Prevention

SRP concentrates on the risks most often linked to deaths, prosecutions and reputational damage in

adult social care:

• Choking and dysphagia

• Scalding and drowning

• Epilepsy related harm, including SUDEP

• Bowel and constipation related harm

• Self-harm and suicide

Crisis Stabilisation

SRP’s Crisis Stabilisation service offers targeted support for organisations facing immediate challenges.

Using proven strategies and insight from our prevention work, we help teams manage urgent situations with confidence.

  • Stabilise the specific risk area to prevent it from happening again.

  • Organise evidence and learning in a way that supports staff, residents and families throughout the investigation and any inquest.

  • Demonstrate to regulators, commissioners and insurers that you are addressing the issue directly, not simply managing reputational damage.
